Dr. Saira Javed is a GP Partner and Trainer at Fountains Medical Practice. She has a passion for medical education. As a GP Trainer she is committed to guiding the next generation of Doctors through their specialty training. Her aim is to deliver high quality, patient centered care while instilling clinical confidence and reflective practice in trainees. With a special interest in Diabetes and chronic disease management, she is committed to supporting individuals in managing their long term conditions through evidence based medicine, education and life style changes. She is a respected member of the Royal College of General Practitioners.

DR SOPHIE HUNT

Dr Hunt works as part of the Care Home Team. She qualified from the University of Liverpool in 2015. During her postgraduate training, she gained additional experience and qualifications in General and Geriatric Medicine. Her qualifications include Membership of the Royal College of Physicians (MRCP), Membership of the Royal College of General Practitioners (MRCGP) and a Diploma in Geriatric Medicine (DGM).

DR SAM MILLS

Dr Mills graduated from Lancaster University in 2017 and has worked as a clinician in the North-West ever since. He became a member of the Royal College of General Practitioners (MRCGP) in 2024 and, alongside General Practice, he has specific experience in urology and community health screening. Dr Mills achieved a postgraduate certificate in medical education (PGCME) in 2021 and teaches medical students at the University of Chester. Outside of medicine, Dr Mills has a love for sports and outdoor activities, including cricket, football and hiking.

By day, Erika is an Advanced Clinical Practitioner; by weekend, you’re more likely to find her climbing a mountain in Wales or the Lakes, or at least attempting to, than sitting still! She’s passionate about women’s health and is a certified British Menopause Practitioner, dedicated to helping women navigate the menopause with confidence and clarity. With postgraduate qualifications in diabetes and cardiovascular medicine, she also supports people to live well with long-term conditions—always with a focus on evidence-based, patient-centred, compassionate care

JAMES LINDLEY

Advanced Clinical Practitioner
James is an Advanced Clinical Practitioner and registered Paramedic working within the Fountains Integrated Care Home Service (FICHS). He has a particular interest in the care of older people, frailty, long-term conditions and advance care planning, with a strong focus on providing compassionate, person-centred care for care home residents. He completed a Master's degree in Advanced Healthcare Practice (Clinical) at Liverpool John Moores University and is also a qualified Independent Prescriber. James is passionate about improving the quality and safety of care in care homes through evidence-based practice, clinical education and service development. Outside of work, James enjoys being on the water, hiking and spending time with his family. He is also an active volunteer within his local community.

Lee joined the practice in September 2021 and has been a Practice Manager since 2009. As the Business Partner he is responsible for all the management aspects of the business.

Operations Manager
Sarah joined the team at Fountains Medical Practice in 2021 as Operations Manager but has worked in Primary Care for the last 12 years and is a qualified Management Professional. Sarah oversees the oversees the operational responsibility and management of the Practice. Sarah enjoys coaching staff, developing individuals to be the best version of themselves and this is a passion for her. In her spare time Sarah enjoys spending time with her family and friends.

Office Manager
Sian joined Fountains Medical Practice in 2020 as an Apprentice on Reception and has progressed through several roles to become Office Manager. Sian oversees the day to day running of the Admin Office at the Practice. Outside of work Sian has a love for outdoor activities with her friends and family such as hiking and wild swimming as well as painting.
