What is FICHS?
For several years the GP practices in Chester have been working to provide improved care to residents in care homes. After careful consideration all the GP practices in Chester agreed for one practice to specialise and look after all the care homes in the city. This new service is called Fountains Integrated Care Homes Service (FICHS).
The service will cover the whole of Chester serving 12 Nursing Homes with approximately 854 residents and 9 Residential Homes with approximately 249 residents. This role out is staggered into 7 phases between June 2020 and June 2021.
Who are FICHS?
FICHS is run by a team of GPs, nurses and other health professionals from Fountains Medical Practice and will aim to deliver proactive and person centred care, keeping in mind the specific health and wellbeing needs of care home patients.

What are the benefits of FICHS?
By exclusively looking after residents in care homes, the service will enable the GPs to spend more time with these patients, and deliver more proactive and specialist care. The team will contact the homes on a weekly basis to conduct routine consultations and meetings, in addition responding to urgent calls when required.
The GPs in this practice have a special interest in Elderly Care and will work closely with the Community Geriatricians, Old Age Psychiatrists and Multidisciplinary teams to provide their expertise within the care homes. This provides an expert service and increased continuity of care.
